If you have a sport physical on file from 2019-20 (completed on or after April 15, 2019), you need to complete the MHSAA Health Questionaire and the Gold Card

Michigan High School Athletic Association (MHSAA) is a service organization for high school sports in Michigan. Similar to NCAA for College Sports, MHSAA is the governing source High School/Middle School sports which exists to develop common rules for athletic eligibility and competition. On the MHSAA site you can find the current season rules (updates and revisions), List of schools in each division (D1 - D3), State Championship Meet information including the link to watch the State Championship Meet (Finals session) Live ($).
